Twelve short modules on the ideas behind Traditional Chinese Medicine — in English and Chinese, with an honest look at the evidence.
This is the overview course: read it straight through in about two hours, or one module at a time. Each module ends with key terms and a short check. If you want to go deeper on any idea, the main 中醫養生 series has more than a hundred lessons.
This course is for education only. It is not medical advice and does not replace a licensed doctor or 中醫師. If you are pregnant, take prescription medicine, or have a health condition, talk to your clinicians before changing anything.
A two-thousand-year-old medical tradition, read with two lenses.
Traditional Chinese Medicine (TCM) is a medical tradition with more than two thousand years of written history. Its foundational text, the Huangdi Neijing (黃帝內經, The Yellow Emperor's Inner Classic), was not written by one person. It was put together by many hands, roughly between the third and first centuries BCE, and it has been commented on, argued over, and revised ever since.
Today it is practiced in clinics and hospitals across the Chinese-speaking world, and related traditions grew up in Korea (Korean medicine, 韓醫) and Japan (Kampo, 漢方). In Taiwan, licensed TCM physicians (中醫師) work alongside Western-trained doctors, many hospitals have a TCM department, and National Health Insurance covers TCM clinic visits.
To read TCM well, you need two lenses at once. The first is the tradition's own logic: what its concepts mean and how practitioners use them. The second is modern evidence: what clinical research has tested, what held up, and what has not been shown. Mixing the two up is the most common mistake, in both directions. Some people dismiss the whole tradition because its theory sounds strange. Others treat every traditional idea as if science had already confirmed it.
This course keeps the two apart. The main text explains what TCM says, in its own terms. The green What research says boxes tell you, as plainly as we can, where the evidence stands.
A TCM diagnosis names a pattern (證, zheng) in the whole person, such as "Spleen qi deficiency with dampness." Two people with the same Western diagnosis, say chronic insomnia, may be given different TCM patterns and different treatments. This approach is called 辨證論治: identify the pattern, then treat.
When TCM says "Spleen," it means a set of functions — mainly turning food into usable nourishment — not the organ you dissected in biology class. Module 4 explains this; it is the key that unlocks almost everything else.
The classics praise the physician who keeps people well over the one who rescues them once they are sick. Diet, sleep, emotion, and living with the seasons are treated as medicine, not as extras.
Therefore the sage does not treat illness that has already arisen, but illness that has not yet arisen; does not put in order what is already in disorder, but what is not yet in disorder.
Not a religion. It borrows words like yin, yang, and qi from Chinese philosophy, but you do not have to believe anything to study it.
Not a replacement for emergency care. A stroke, a heart attack, a broken bone, or a high fever in a baby needs a hospital first.
Not "natural, therefore safe." Herbs are active substances. Some are toxic, and some interact with prescription drugs (Module 11).

Quick check
1.Who wrote the Huangdi Neijing?《
The Yellow Emperor is a legendary figure the text is framed around. The book was compiled by many hands, roughly in the third to first centuries BCE.
2.Two patients have the same Western diagnosis. Will a TCM physician treat them the same way?
This is 辨證論治: treatment follows the pattern, so the same disease can get different treatments (同病異治).
3.When TCM talks about the "Spleen," it mainly means…
TCM organs are systems of function. The TCM Spleen governs "transformation and transportation" (運化) of food and fluids.
4.Which statement about the evidence for TCM is most accurate?
Evidence has to be judged therapy by therapy and condition by condition — acupuncture for chronic pain is not the same question as an herb for diabetes.
5.Your friend has sudden chest pain spreading to the left arm. What should they do first?
Possible heart attack. Emergencies go to the hospital first; TCM can have a role later, in recovery.
Not mysticism, and not good versus bad — a language for describing balance.
The characters tell the story. 陰 (yin) first meant the shady side of a hill; 陽 (yang) meant the sunny side. Same hill, two sides, and which side is which depends on where the sun is. Yin and yang are not two substances. They are relational labels: anything can be called more yin or more yang compared with something else.

Four relationships
Opposition對立
They pull in opposite directions and keep each other in check — heat checks cold, activity checks rest.
Interdependence互根
Neither exists alone. Night only means something because there is day; activity needs the rest that makes it possible. A saying from many centuries later puts it: 孤陰不生,獨陽不長 — yin alone cannot give life, yang alone cannot grow.
When one grows, the other shrinks, like the length of day and night across the year.
Transformation轉化
At an extreme, each turns into the other. The longest day of the year is the start of days getting shorter. The small dots in the taiji symbol show the seed of each inside the other.
In TCM, health is a working balance between yang — the body's warming, moving, transforming functions — and yin — the cooling, moistening, nourishing substances that anchor them. When the balance tips, the tradition describes four basic patterns.
hot palms and soles, night sweats, dry mouth at night手足心熱、盜汗、夜間口乾
Notice that yang excess and yin deficiency can both make you feel hot — but in TCM logic the cause is opposite. One is too much heat; the other is too little cooling. The first is "cleared," the second "nourished." This is why the tradition warns against self-treating from a symptom alone.
Yin and yang are relative labels for two sides of one balance. In the body, the question TCM asks is not just "hot or cold?" but "too much of one, or too little of the other?"
Why "energy" is the wrong translation — and what qi does instead.
Qi (氣) is usually translated as "energy," and that translation causes trouble. Energy, in physics, is a quantity you can measure and store. Qi is not a substance sitting in a tank. In TCM, qi is better understood as the body doing its work: moving, warming, defending, holding things in place, and transforming food and air into the body's own substances. When a TCM physician says your qi is weak, they mean those functions are running below full strength — not that a battery is low.
Many teachers now leave qi untranslated, the way English keeps karma and nirvana. This course does the same.
The five functions of qi
Propelling推動
Moves blood and fluids, and drives growth and the work of every organ.
Warming
Keeps the body warm. Cold hands and feet are often read as qi (or yang) not reaching them.
Defending防禦
Defensive qi (衛氣) guards the surface of the body against outside "pathogens" such as wind and cold.
Containing固攝
Holds blood inside the vessels, sweat and urine until the right time, and organs in place.
Transforming氣化
Turns food, drink, and air into qi, blood, and fluids, and turns waste into sweat and urine.
Where qi comes from氣從哪裡來
Inherited qi
What you are born with, said to be stored in the Kidney. You cannot add to it, only use it wisely.
Food qi
Extracted from food and drink by the Spleen and Stomach — the source you control most.
Clear qi清氣
Drawn in by the Lung with each breath.
Four ways qi goes wrong
Pattern型態
What the tradition describes
Qi deficiency
tired, soft voice, short of breath on effort, sweats easily, catches colds often疲倦、說話無力、動則氣喘、容易出汗、常感冒
Qi stagnation
distension that comes and goes, sighing, tightness in the chest or sides, worse with stress時脹時消、常嘆氣、胸脅悶脹、壓力大時加重
Sinking qi
a dragging-down feeling, prolapse, fatigue that is worse standing下墜感、臟器下垂、站久更累
Rebellious qi
qi going the wrong way: burping, hiccups, reflux, nausea, coughing氣往反方向走:打嗝、呃逆、胃酸逆流、噁心、咳嗽
Take with you帶走這一句
Qi is not fuel in a tank; it is the body at work. TCM asks two different questions: is there not enough of it (deficiency), or is it stuck or going the wrong way (stagnation, rebellion)?
The key that unlocks the rest: what "Spleen," "Liver," and "Kidney" mean in TCM.
Early Chinese physicians did look inside the body, but the organ names in TCM came to stand for clusters of function observed from the outside: what happens when you eat, breathe, sleep, get angry, or grow old. So when this course writes "Spleen" or "Liver" with a capital letter, read it as "the TCM Spleen system," not the organ in an anatomy textbook.
governs blood and vessels; houses the mind
only the pump in your chest只是胸口那顆幫浦
Liver肝
keeps qi flowing smoothly (); stores blood主疏泄、藏血
the liver's chemistry on a blood test抽血報告上的肝功能
Spleen脾
transforms and transports food and fluids (運化); keeps blood in the vessels主運化、統血
the spleen, which filters blood負責過濾血液的脾臟
Lung肺
governs qi and breathing; spreads defensive qi to the skin; regulates water主氣司呼吸、宣發衛氣、通調水道
only the lungs只是兩片肺葉
Kidney腎
stores essence (精) for growth, reproduction, and aging; governs water; grasps qi藏精、主水、主納氣
the kidneys, which filter urine製造尿液的腎臟
Each zang organ is paired with a fu (腑) organ: Liver with Gallbladder, Heart with Small Intestine, Spleen with Stomach, Lung with Large Intestine, Kidney with Bladder. The fu organs are "hollow": they receive, pass along, and empty out. A sixth fu, the Triple Burner (三焦), has no single anatomical counterpart at all; it names the pathways of water and qi through the trunk.
Because food qi is the source you control every day (Module 3), the Spleen and Stomach are called "the root of later heaven" (後天之本) — the foundation you build after birth. Much TCM advice about warm, cooked, regular meals is really advice about protecting the Spleen. The tradition says the Spleen likes dryness and warmth and dislikes dampness and cold; that is why a large iced drink on an empty stomach is a classic thing TCM tells you to avoid.
A TCM organ name is a label for a set of functions. Learn the functions, and the rest of TCM starts to make sense — but never use a TCM organ diagnosis in place of a blood test.
Wood, Fire, Earth, Metal, Water: a map of how systems feed and check one another.
The five xing (五行) are often called the "Five Elements," but 行 means to move or to act, so many scholars prefer "Five Phases." They are not chemical elements. They are five categories for sorting how things behave: Wood grows and spreads, Fire rises and flares, Earth holds and nourishes, Metal gathers and contracts, Water sinks and stores.
This interactive part needs JavaScript.
Two cycles
Generating cycle相生
Each phase nourishes the next, like a mother feeding a child: Wood → Fire → Earth → Metal → Water → Wood. Wood feeds fire; fire leaves ash, which becomes earth; earth yields metal; metal gathers water (think of dew on cool metal); water nourishes trees.
Each phase restrains another, so nothing runs away: Wood → Earth → Water → Fire → Metal → Wood. Roots break up soil; earth dams water; water puts out fire; fire melts metal; an axe cuts wood.
A classic strategy is called 培土生金, "bank up Earth to generate Metal." Someone with weak digestion (Earth, the Spleen) who also catches colds easily (Metal, the Lung) may be treated mainly by strengthening digestion, on the reasoning that the mother phase feeds the child. This is how the five phases are used in practice: as a way of reasoning about connections, so that a physician looks beyond the organ where the symptom appears.
The five phases are a relationship map. Their value in TCM is the habit of thinking in connections — what feeds this, and what is supposed to keep it in check?
Your usual tendencies, according to the nine-constitution model — a mirror, not a diagnosis.
TCM pays attention not only to what is wrong today, but to the kind of body you have: the direction you tend to drift when you are tired, stressed, or eating badly. This is your constitution (體質, tizhi). Two people can catch the same cold and react quite differently, and TCM would say part of the reason is constitution.
The best-known modern system was developed by Professor Wang Qi (王琦) of the Beijing University of Chinese Medicine. It describes nine constitutions and became a standard of the China Association of Chinese Medicine in 2009. Researchers assess it with a 60-item questionnaire. Most people are not one pure type; they have a main tendency and often a second one.
This interactive part needs JavaScript.
Constitution is described as a starting point, not a destiny. In the tradition it shifts slowly with age, diet, sleep, climate, and emotional life — which is the whole reason TCM gives so much daily-life advice.
Constitution is your body's usual direction of drift. Knowing the nine types helps you understand TCM advice — but a checklist is not an assessment, and a new or worsening symptom is a reason to see a doctor.
Every food has a "nature" and a "flavor" — and cooking changes both.
A classical phrase says 藥食同源: medicine and food come from the same source. TCM draws no sharp line between the kitchen and the pharmacy; ginger, scallions, Chinese yam, and red dates all appear in classical formulas. The aim of eating well, in TCM terms, is to choose foods whose properties balance your body, the season, and the climate you live in.
Foods are sorted on two scales. Nature (性) runs cold – cool – neutral – warm – hot and describes the food's effect on the body, not its temperature on the plate: watermelon is cold even at room temperature, and ginger is warm even in an iced drink. Flavor (味) has five values — sour, bitter, sweet, pungent, salty — each said to act in a particular way.
This interactive part needs JavaScript.
Cooking and pairing change the picture
Cooking warms. Stir-fried cucumber is less cooling than raw; stewing and roasting add warmth.
Pairing balances. Crab (cold) is eaten with ginger; hot pot is balanced with cooling vegetables; cold tofu gets scallions and ginger.
Season and climate matter. The same bowl of mung bean soup that suits a Taiwanese August is a poor choice on a cold January night.
Moderation is the rule. The tradition warns that too much of any one flavor harms its own organ — too much sweet burdens the Spleen.
Take with you帶走這一句
Three things are worth remembering: cooked is warmer than raw, pairing can balance an extreme food, and the right food depends on your body and the season.

Watermelon is cold even at room temperature; ginger is warm even iced.
2.Why is crab traditionally eaten with ginger?
Pairing an extreme food with its opposite is a core idea of TCM cooking.
3.What does cooking generally do to a food's nature?
Stir-frying, stewing, and roasting add warmth.
4.Which flavor is said to tonify and harmonize?
Sweet foods like rice and sweet potato — in moderation. Too much sweet burdens the Spleen.
5.TCM says "avoid raw salads" for a cold-natured person. What fits both TCM and nutrition advice?
You keep the nutrition and follow the tradition's logic at the same time.

The Neijing devotes a whole chapter (Suwen, Chapter 2) to living in step with the seasons. The principle is to go in the same direction as the season's movement: grow in spring, flourish in summer, gather in autumn, store in winter.
The yin and yang of the four seasons are the root of all things. So the sage nourishes yang in spring and summer, and yin in autumn and winter, to follow that root.

The organ clock
You will often see a "TCM body clock" that gives each of the twelve main channels a two-hour peak: Lung from 3 to 5 a.m., Large Intestine 5 to 7, Stomach 7 to 9, and so on around the day. Its roots are old — the Neijing already describes qi flowing through the channels in a fixed order, but gives no clock times. The two-hour clock appears in later acupuncture texts — for example Dou Hanqing's Zhenjing Zhinan (針經指南, Yuan dynasty, 1295) — and is often taught under the name 子午流注, a term that also covers older methods for choosing points by day and hour.
A map of connections — and five points you can press yourself.
Meridians, or channels (經絡, jingluo), are the pathways along which TCM says qi and blood circulate, linking the surface of the body to the organs inside. There are twelve principal channels, each named for an organ, plus eight "extraordinary vessels" — the best known are the Governing Vessel (督脈) up the spine and the Conception Vessel (任脈) down the front midline.
Along the channels lie the acupuncture points (穴位). In 2008 the World Health Organization published standard locations for 361 of them, using codes like LI4 (Large Intestine 4) and ST36 (Stomach 36), so that practitioners and researchers around the world mean the same spot.

Acupressure — firm pressure with a thumb or fingertip — is the self-care version of acupuncture. Press straight into the point for 30 to 60 seconds, or rub in small circles for a minute or two, firmly enough to feel a dull ache but not sharp pain. Do both sides. The traditional unit of measurement is the cun (寸), based on your own body: the width of four fingers held together is about 3 cun.

The Chinese word 針灸 (zhenjiu) joins two therapies: 針, needling, and 灸, moxibustion — warming a point with burning mugwort (艾草). Modern acupuncture needles are nothing like injection needles. They are solid, flexible, and very thin (roughly 0.16 to 0.30 mm), sterile, and used once. Most people feel a brief prick, then a dull, heavy, or spreading ache called de qi (得氣).
Assessment. Questions about your history, sleep, and digestion; looking at your tongue; feeling the pulse at both wrists.

Needling. Usually several to a dozen or more needles, chosen for your pattern, sometimes far from where it hurts.

Formulas, not single herbs — and why "natural" is not the same as "safe."
Chinese herbal medicine is TCM's deepest clinical tool, with a written tradition running from the Shennong Bencao Jing (神農本草經, Eastern Han dynasty, 365 substances) to Li Shizhen's great Bencao Gangmu (本草綱目), finished in 1578 and printed in the 1590s. Most substances are plants; some are minerals or animal products.
Unlike much Western herbalism, TCM rarely uses one herb alone. It uses formulas (方劑) of several herbs with assigned roles — the "sovereign, minister, assistant, and envoy" (君臣佐使). The sovereign targets the main problem; the minister supports it; the assistant handles side issues or tempers harshness; the envoy guides and harmonizes the whole.

Safety: what every reader should know安全:每個人都該知道的事
Herbs are drugs. Some are toxic. Taiwan banned herbs containing aristolochic acid in 2003 after they were linked to kidney failure and urinary-tract cancers — a lesson in why "traditional" is not proof of safety.
Interactions are real. Dong quai (當歸) and danshen (丹參) may raise bleeding risk with warfarin; American ginseng can weaken warfarin's effect. Tell both your TCM physician and your Western doctor everything you take.
Don't self-prescribe formulas. A tonic for the wrong pattern can make things worse — the tradition itself warns that ginseng for someone with damp-heat can aggravate heat signs.

These are classified as both food and medicine and are reasonable for everyday cooking in normal amounts: fresh ginger (生薑), red dates (紅棗), goji berries (枸杞), dried tangerine peel (陳皮), and Chinese yam (山藥). Anything beyond the kitchen — capsules, concentrated extracts, or a formula — is a conversation for a licensed practitioner.
Chinese herbs are real pharmacology: powerful, formula-based, and capable of harm. Cook with kitchen herbs freely; for anything more, see a licensed practitioner and keep your Western doctor informed.
