Unit 101: The Four-Layer Model for How a Fever Progresses

This course has, across several lessons, treated qi and blood as substances the body produces and relies on. A framework called "defense, qi, nutritive, blood" puts those same categories, plus a fourth layer, to a different use: mapping how a warm-type illness is traditionally understood to progress through the body in four increasingly deep stages, rather than describing what qi and blood do when the body is healthy.

這門課在好幾課裡,都把氣跟血當成身體產生、並且依賴的物質。一個叫做「衛氣營血」的框架,把同樣這些類別,加上第四層,用在一個不同的地方:畫出一個溫病傳統上被理解成如何以四個逐漸加深的階段,在身體裡推進,而不是描述身體健康時氣跟血在做什麼。

The four stages run from surface to core. The defense-level stage is the shallowest, corresponding to early symptoms like fever and chills as a pathogen first meets the body's surface defenses. If unresolved, illness is said to progress to the qi-level stage, marked by higher fever without chills, as the pathogen moves past the surface. Beyond that lies the nutritive-level stage, where disturbed sleep and a reddened tongue are said to appear as the pathogen reaches deeper, more vital tissue. The blood-level stage is the deepest and most serious, traditionally associated with bleeding or severe agitation, as the pathogen affects blood itself.

這四個階段,從表層一路到核心。衛分是最淺的階段,對應到發燒與惡寒這類早期症狀,這是病邪剛接觸到身體表層防禦的時候。如果沒有被解決,病情被說成會推進到氣分階段,特徵是發燒加重、但不再惡寒,因為病邪已經越過了表層。再往裡是營分階段,睡眠被擾亂、舌頭發紅,被說成是病邪到達更深、更重要的組織時會出現的徵象。血分階段是最深、也最嚴重的,傳統上跟出血或嚴重躁動連結在一起,因為病邪這時已經影響到血本身。

The practical value of the four-layer model is that it turns an otherwise vague question — "is this getting worse?" — into a specific one: which of the four layers is a given set of symptoms consistent with, and is the pattern of symptoms moving toward the surface or toward the core. This is one more example of a theme running through the whole course: Chinese medicine consistently prefers naming a specific stage or direction over describing severity in only vague terms like "mild" or "serious."

衛氣營血這個模型實際的用處,是把一個原本模糊的問題——「這個是不是變嚴重了?」——變成一個具體的問題:眼前這組症狀,符合四層裡的哪一層,而症狀的模式,是正往表層移動、還是正往核心移動。這是貫穿整門課的一個主題的又一個例子:中醫一貫偏好指出一個具體的階段或方向,而不是只用「輕微」或「嚴重」這種模糊的字眼來描述嚴重度。
